I think it is intentionally left out to deter the driver from removing both hands from the wheel...

They have the handles on the driver side to help get in the truck. Unless you have a lift kit on your vibe, I don't see why youd want a handle when your supposed to be driving.

My wife's Park Avenue Ultra also has only 3. Guess they assume the driver is not a complete feeb. Bad assumption!

This is probably more than likely done as a safety feature so the driver keeps their hands on the wheel where it is supposed to be... not on an overhead handle. In regards to a truck, I can see use for them being higher off the ground, but for a passenger vehicle, nah.

I would expect there to be mounting holes. In the world outside North America we are sitting on the passenger side. They possibly just made a universal headliner it fit all of the vibes/matrixes regardless of where they are destined to be sold.

I think pwarren4 nauiled it. But I'll add that it probably saved them a dollar or two to NOT put one in. In my Nissan Altima, one D-ring was missing from the trunk for nets to hook into. I complained to my dealer and he put the missing one in. But it turns out they were missing from every Altima. And for every person who didn't notice and didn't complain, they saved a couple of dollars.KSNeptune
