Michigan Rally ID Forms For Confirmed Attendees

I have put together a form that we can put in the window of our cars at our Genvibe rally in June. It will help with names and stuff you have done with your car. See the attatchment. If you have confirmed that you will be there or are over 90% sure you will be there, please email me at smoothop45@hotmail.com with the following information:Genvibe member name:Real name:Where you are from: (city, state)Car nickname: (if you have one)Car options:Current mods:Future mods:I will have all of the forms made before the rally and you will pick them up from me there. I will have some extra blank forms made so if someone is able to come that didn't know if they were going to make it, they could hand write the fields.If you have any suggestions for the form, please let me know. I would be happy to make some changes if needed. Here is the form:
