MOTM questionnaire suggestions?

Hey folks, with this being my first month running MOTM I thought I'd leave my little thumb print by changing up and/or expanding the questionnaire that the winner fills out.So my question is, what questions would you all like to see (beyond the basic name, rank and serial number)?
