Transmission Code P0741

Hello everyone, I have an 04 base Vibe with 272,000 miles on it. I have had a P0420 code since I bought the car so I occasionally check the codes to make sure nothing new has popped up. This morning I found code P0741. I don't have any driveability issues at the moment but I was wondering if my transmission fluid might be the reason for the code. From what I know off of maintenance records the transmission fluid is the original fluid from the factory. I check the fluid every 3 months and the level is within its normal range but I know when the fluid is that age and with that many miles it can lead to some issues. What should I do to clear the code? Should I try and do a fluid & filter change?
